Screen your patient

CHA₂DS₂-VASc score ≥2 (males) or ≥3 (females)

Labile INR or inability to maintain time in therapeutic range (TTR <60%)

Alcohol use (≥ 8 drinks/week)

Intracranial bleeding

Spontaneous major bleeding other than intracranial

Minor bleeding

Intolerant or experiences side effects from oral anticoagulation, not including bleeding

Difficulty taking their oral anticoagulant consistently as prescribed

Unable to keep regular INR blood testing appointments

Bleeding risk from health conditions not reflected by HAS-BLED score

Decreased stability or mobility that may increase risk of falling

Has a job or hobby that increases the risk of falling or bleeding

Made lifestyle tradeoffs due to taking oral anticoagulation to maintain their quality of life

Indicated a preference for an alternative to their prescribed OAC, such left atrial appendage closure, to maintain their quality of life.

Other: Meets another clinical criterion that was not previously specified that may make OACs inappropriate.
